Access is issued, not self-serve from the website

Chrisalys does not offer an open patient-portal login on this website. If you are an existing patient, use the secure link and instructions you received from the care team. If that message is buried, call (310) 247-4729 and we will resend it to the email we have on file. We will not reset access in a public chat box.

If you are not yet a patient, a portal account will not appear because you visited this URL. Schedule a consultation or start pre-authorization instead. Forms for new patients are sent after you are in the system.

We take records requests seriously. HIPAA rights are described in our privacy notice. Portal messages are not for emergencies. For urgent post-op problems, use the after-hours number on your discharge papers or call 911.

Book a consult or insurance intake first. Portal access follows enrollment, not the other way around.

Urgent issues

Do not wait on a portal reply for bleeding, fever, or trouble breathing. Call emergency services or the number on your post-op sheet.

I never got a portal invite. Can I create an account here?

No. We issue access. Call the office and we will send the link to the email in your chart.

Can I send records through this page?

Not here. Once you are a patient, the care team will tell you how to upload. Referring providers: /referring-providers.

Is the portal the same as the consult form?

No. The consult form on this site is for people who are not yet scheduled. The portal is for people already in our care.
