We would rather get a complete packet once

Chrisalys Surgery accepts referrals from primary care, hormone prescribers, therapists, and other surgeons. We are a trans-only multidisciplinary practice: face, voice, top, bottom, body, and peri-operative therapy. You do not need to find three separate gender programs in Los Angeles for core procedures.

Please send: the clinical question (which procedures), hormone timeline, relevant operative history, mental-health letters if they already exist, and insurance information including member ID and whether the product is Medi-Cal, LA Care, IEHP, or commercial. If the plan requires a named surgeon of record, call (310) 247-4729 before the referral goes out so the name matches the case.

Unless you have asked us to wait. Please tell the patient you referred them so our call is not a surprise.

We often start pre-auth from our side. Your office may still need to click a portal. We will say when.

Do you require a physician referral?

We do not, unless the patient's plan does. HMOs often do. We will tell the patient and you if that is the blocker.

Which surgeon should appear on the referral?

Face and body: often Dr. David N. Sayah, MD. FFS and voice: Dr. Toby Mayer. Genital: Dr. Maurice Garcia. Call us if you need the exact surgeon of record for a referral.

Can I send records without a patient-signed release?

Follow HIPAA. A treatment referral is a permitted path in many cases; we still prefer a packet the patient knows about. Our notice: /hipaa.
